Understanding BMI

What is Body Mass Index?

BMI is the most widely used screening tool to assess whether your weight is in a healthy range for your height. It’s used by the NHS, doctors, and bariatric surgeons worldwide as a starting point for weight assessment.

How BMI Works

Body Mass Index (BMI) is a simple calculation that uses your height and weight to give a single number representing your body composition. While not perfect, it provides a quick reference point that correlates well with body fat for most adults.

The NHS, World Health Organization, and bariatric societies all use BMI as the primary criterion to determine eligibility for weight loss surgery, lifestyle interventions, or medical treatment.

Important: BMI is a screening tool, not a diagnosis. It works best for sedentary adults aged 18–65 with average muscle mass. Athletes, pregnant women, and elderly people may need additional measurements.

The BMI Formula
BMI = Weight (kg) Height (m)2
Example: 70 kg ÷ (1.70 m × 1.70 m) = 24.2 BMI (Healthy Weight)

What Does Your BMI Score Mean?

BMI categories help you understand where you stand and what action might be appropriate. Each range carries different health implications and treatment recommendations.

Over
Obese I
Obese II+
<18.5
18.5 — 24.9
25 — 29.9
30 — 34.9
35+

Underweight

BMI Below 18.5

A BMI below 18.5 may indicate undernutrition, an underlying health condition, or a restrictive eating pattern. Being underweight can weaken immunity, affect bone health, and reduce energy levels.

Consult your GP for assessment

Healthy Weight

BMI 18.5 — 24.9

You’re in the healthy range. Maintain a balanced diet and regular exercise. If you’ve recently lost significant weight, you may benefit from body contouring procedures to address loose skin.

Maintain & consider body contouring

Overweight

BMI 25 — 29.9

Slightly above healthy range. Increased risk of type 2 diabetes, heart disease, and joint problems. Lifestyle changes can be effective. The Allurion gastric balloon is ideal at this BMI — non-surgical and removed naturally.

Allurion balloon may be ideal

Obese Class I

BMI 30 — 34.9

You may qualify for bariatric surgery in Turkey. Gastric sleeve is the most popular procedure for this range and typically delivers 60–70% excess weight loss within 12–18 months.

Gastric sleeve eligibility

Obese Class II

BMI 35 — 39.9

Strong candidate for bariatric surgery. Gastric sleeve or gastric bypass can resolve obesity-related conditions including type 2 diabetes, hypertension, and sleep apnoea. Surgery is typically recommended at this BMI.

Sleeve or bypass recommended

Obese Class III

BMI 40+

Severe obesity. Bariatric surgery can be life-saving and significantly improve life expectancy. Gastric bypass is typically the most effective option at this BMI, achieving 70–80% excess weight loss long-term.

Gastric bypass strongly advised
Eligibility Comparison

NHS vs Turkey: Bariatric Surgery Criteria

The eligibility criteria for weight loss surgery differ significantly between the NHS and private clinics in Turkey. Understanding these differences can help you assess your options.

NHS UK Criteria

Strict & Long Wait

NICE guidelines for bariatric surgery on the NHS

  • BMI 40+ required Or BMI 35+ with serious obesity-related condition (e.g., type 2 diabetes)
  • Try non-surgical options first Must complete tier 3 weight management programme (12+ months)
  • GP referral required Multiple specialist appointments and assessments needed
  • Long waiting times Average wait: 18–36 months from referral to surgery
  • Limited surgery options Mostly gastric bypass; sleeve and balloon often unavailable on NHS

NewMe Turkey Criteria

Flexible & Fast

International standards used in Turkey’s top clinics

  • BMI 30+ accepted Or BMI 27+ with obesity-related conditions like diabetes or hypertension
  • No prior programme required Direct access to surgical assessment and treatment planning
  • Free consultation WhatsApp or video call — no GP referral needed
  • Surgery within 2–4 weeks Schedule your procedure based on your availability
  • Full range of options Gastric sleeve, bypass, Allurion balloon, revisional surgery
2–4 weeks

From consultation to surgery

Why is the criteria more flexible in Turkey?

The NHS has limited resources and must prioritise the most severe cases. Private clinics in Turkey follow international standards (similar to private UK clinics) and accept patients earlier in their obesity journey, when intervention is often more effective and prevents serious complications.

BMI Limitations: When the Number Doesn’t Tell the Full Story

BMI is a useful screening tool, but it has well-known limitations. Understanding when BMI may be inaccurate helps you make better health decisions.

Doesn’t distinguish muscle from fat

Athletes and bodybuilders often have a high BMI due to muscle mass, not fat. A 95 kg muscular person at 1.80 m has a BMI of 29.3 (overweight) but may have low body fat.

Doesn’t account for fat distribution

Two people with the same BMI can have very different health risks based on where they carry their fat. Abdominal (visceral) fat is more dangerous than fat on hips or thighs.

Less accurate for older adults

People aged 65+ naturally lose muscle and gain fat, making BMI less reliable. Some research suggests slightly higher BMI in older adults may actually be protective.

Ethnicity matters

Asian populations face health risks at lower BMI thresholds (BMI 23+ may be overweight). Black and African-Caribbean populations may be metabolically healthy at higher BMI levels.

Not for pregnant women

Weight gain during pregnancy is normal and necessary. BMI shouldn’t be used during pregnancy or in the months immediately after childbirth.

One number, multiple meanings

BMI doesn’t measure metabolic health, blood pressure, cholesterol, or fitness. Two people with the same BMI can have very different overall health profiles.

BMI is a starting point, not a diagnosis. When assessing patients for bariatric surgery, we look at the complete picture — waist circumference, body composition, blood markers, lifestyle, and obesity-related conditions. Two patients with the same BMI may need very different approaches.

FAQ

Frequently Asked Questions About BMI

Common questions about Body Mass Index, weight loss surgery eligibility, and what your BMI means for your health.

What is a healthy BMI for adults?

For adults aged 18 and over, a healthy BMI is between 18.5 and 24.9. Below 18.5 is considered underweight, 25–29.9 is overweight, and 30 or above is classified as obese. These ranges are used by the NHS, WHO, and most international medical bodies.

Note that ethnicity can affect ideal BMI ranges. People of Asian descent may face health risks at BMI 23+, while some Black and African-Caribbean populations may remain metabolically healthy at slightly higher BMIs.

What BMI do I need for gastric sleeve surgery in Turkey?

At NewMe Health Clinic, we accept patients with a BMI of 30 or higher for gastric sleeve surgery, or BMI 27+ if you have an obesity-related condition like type 2 diabetes, hypertension, or sleep apnoea. This is more flexible than NHS criteria, which typically requires BMI 40+ or 35+ with comorbidities.

Final eligibility is determined during your free consultation, which includes review of your medical history and overall health status.

Can I have weight loss surgery with a BMI below 30?

Yes, in some cases. If your BMI is between 27 and 29.9 and you have obesity-related conditions (type 2 diabetes, high blood pressure, sleep apnoea, fatty liver disease), you may qualify for bariatric surgery.

For BMI 25–29.9 without comorbidities, the Allurion gastric balloon is often a better option — it’s non-surgical and can help you lose 10–15 kg without permanent stomach modification.

Why does the NHS require BMI 40+ when private clinics accept BMI 30+?

The NHS has limited resources and must prioritise the most severe cases. With long waiting lists and budget constraints, only patients with BMI 40+ (or 35+ with comorbidities) typically meet the threshold for surgery on the NHS.

Private clinics in Turkey follow international standards (similar to private UK clinics) where surgery is considered preventive medicine. Earlier intervention often produces better outcomes and prevents serious obesity-related diseases from developing.

Is BMI accurate for muscular people or athletes?

No. BMI doesn’t distinguish between muscle and fat. A muscular athlete may have a BMI in the ‘overweight’ or even ‘obese’ range while having a low body fat percentage. For these individuals, body fat measurement, waist circumference, and physical fitness assessments are more accurate.

If you exercise regularly with significant muscle mass, ask your GP or specialist for a more comprehensive body composition assessment.

How do I calculate BMI manually?

Metric formula: BMI = weight (kg) ÷ height (m)2

Example: A person weighing 80 kg with height 1.75 m: 80 ÷ (1.75 × 1.75) = 80 ÷ 3.0625 = 26.1 BMI (Overweight).

Imperial formula: BMI = (weight in pounds ÷ height in inches2) × 703

Or simply use our calculator above — it works in both metric and imperial units.

What’s the difference between BMI and body fat percentage?

BMI uses only height and weight to give a quick estimate of healthy weight range. Body fat percentage measures the actual amount of fat in your body using methods like skin-fold callipers, bioelectrical impedance, or DEXA scans.

BMI is faster and free, while body fat percentage is more accurate but requires equipment. For most adults, BMI is a good starting point. If you’re considering weight loss surgery, your specialist will likely measure both.

Does BMI work for children and teenagers?

BMI for children and teenagers (ages 2–17) uses age and gender-specific percentiles, not the adult ranges. This calculator is designed for adults aged 18 and over.

If you’re concerned about a child’s weight, please consult their GP or use the NHS BMI calculator for children. Bariatric surgery is generally not recommended for under-18s except in extreme cases under specialist supervision.

How much weight will I lose with bariatric surgery?

Weight loss varies by procedure:

  • Allurion gastric balloon: 10–15 kg over 16 weeks
  • Gastric sleeve: 60–70% of excess weight (typically 30–50 kg) within 12–18 months
  • Gastric bypass: 70–80% of excess weight within 18–24 months

Long-term success depends heavily on lifestyle changes, dietary compliance, and regular exercise. NewMe provides 1–5 year aftercare support to help you maintain results.

What if my BMI is borderline (e.g., 29.9 or 30.1)?

Borderline BMI values shouldn’t be the only deciding factor. We assess your complete health profile including waist circumference, blood pressure, cholesterol levels, insulin resistance, family history, and quality of life impact.

If you’re close to a treatment threshold, book a free consultation. We’ll review your case honestly and recommend the most appropriate option — whether that’s lifestyle changes, the Allurion balloon, or surgical intervention.

How long does it take to lose weight after surgery?

Most weight loss happens in the first 6–12 months after bariatric surgery, with continued (slower) loss over the next 12–18 months. Total time to reach goal weight varies but is typically:

  • Gastric sleeve: 12–18 months for full weight loss
  • Gastric bypass: 18–24 months for full weight loss
  • Allurion balloon: 16 weeks with balloon + 8 weeks aftercare

Does my BMI need to drop before surgery?

For most patients, no. However, for patients with very high BMI (above 50–55) or significant fatty liver, we may recommend a 2–4 week pre-operative liver-shrinking diet to improve safety during surgery.

Your surgeon will assess this during your consultation. If pre-op weight loss is needed, we’ll provide a specific dietary plan.

Can BMI go too low after surgery?

It’s rare but possible. Some patients lose more weight than expected and reach a BMI in the underweight range. This typically happens 12–18 months post-surgery.

This is why our 1–5 year aftercare programme includes nutritional monitoring, vitamin supplementation guidance, and dietary support to help you stabilise at a healthy weight.
